Low calorie diets of 1000-1200 Calories per day for women and 1200-1600 Calories per day for
men are currently recommended by the National Institutes of Health for weight loss in most
individuals. The rationale is that on such regimens a deficit of approximately 500-1000 Calories
per day will be created, which should result in slow, progressive weight loss of 1-2 pounds per
week.
It is important to recognize that when using fixed 1200 to 1600 Calorie reducing diet plans such as
these that even with perfect adherence, individuals can be expected to vary greatly in their weight
loss. This is because their resting energy needs and physical activity, and thus their energy
outputs, often differ markedly.
When using any of LCDs, maintaining a protein intake of at least 1 g/kg of high–biologic-value
protein is vital to preserve lean body mass. The major potential complications to watch for in
patients on these restricted diet plans include vitamin deficiency states, starvation ketosis,
electrolyte derangements, and cholelithiasis. Although useful for short-term weight loss, none of
these dietary regimens alone is associated with reliable sustained weight loss.

Most patients will lose weight on 1200 kcal (5000kJ) daily and all will lose on a 1000 kcal (4200kJ)
diet. A pragmatic approach is to place all patients on 1000-1200 kcal daily and maintain this if
satisfactory weight loss is continued (2-3 kg/month). Such a 1200 kcal diet should contain 65g
protein, 50 g fat, 130 g carbohydrate and adequate quantities of vitamins and minerals. The diet is
generally better tolerated as 3-4 small meals per day than as one or two larger meals. If
compliance is complete this usually adequate therapy, but even in the absence of cheating,
occasional patients lose less than 1 kg per month. If this happens, intake may be further
decreased to 800kcal (3300kJ) daily. On 800 kcal daily, occasional compliant patients still lose
only 1-2 kg per month, but they should be encouraged to persevere new weight. Diets of less than
800 kcal should not in general be employed for out-patient treatment, except by specialist centers
with careful monitoring.
