Causes of Obesity

The pathogenesis of obesity is far more complex than the simple paradigm of an imbalance
between energy intake and energy output. Although this concept allows easy conceptualization
of the various mechanisms involved in the development of obesity, obesity obviously is far more
than the net result of excess eating and/or too little exercise.

Obesity tends to run in families. However, families share not only genes but also environment
as diet and lifestyle habits and separating the two influences is difficult.

Genetic factors explain about one third to two thirds of the variability in body weight. Also
environmental factors influence body weight, but precisely how they interact with genetic factors
is unclear.

One proposed explanation is that body weight is regulated around a set point, similar to a
thermostat setting. Some people may have a higher-than-normal set point, which may explain
why they are obese and why losing weight and maintaining weight loss are difficult for them.
